Glucose tracker app: a home log in mmol/L or mg/dL

Free printable blood sugar log (PDF)

A glucose tracker app stores readings from a meter you already own. It does not prick a finger, and it is not a continuous glucose monitor. Blood Snap is a free diary for those numbers, next to blood pressure and weight if you log those too.

What to record

  • The glucose value, in the unit you chose (mmol/L or mg/dL).
  • Context: fasting, before meal, after meal, or bedtime.
  • The time. A number without a time is hard to discuss later.

Day-to-day logging is separate from an A1C lab test. See A1C vs a home glucose log and how to log blood sugar at home.

Meter first, app second

Use your glucometer, including Accu-Chek or Contour, the way its manual describes. Then enter the number or snap the display and confirm it before save. Bluetooth is optional and often the step that fails; the diary still works when you type. Related: Accu-Chek without Bluetooth.

Paper sheet or the app

A short paper run fits on the free blood sugar chart printable. When you want reminders and a PDF of more than two weeks, use the blood sugar tracker app. Charts show values you logged. They are not a diagnosis.

Educational information. Not medical advice. Blood Snap is not a glucometer, glucose meter, or CGM. Always follow your clinician's advice.

Questions about this guide

It is a diary for glucose numbers you already measured. You type, speak, or photograph a meter display, tag fasting or after a meal, and keep a chart. It does not replace a glucometer or a CGM.

Log the numbers you just read about.
